The Nice Guys is an All-Volunteer Charity and receives many requests every month.  In order for us to accept your request and begin processing and vetting, we do require you to follow one of the 2 options below.  You must fill out the required forms completely and mail to

When your request for assistance is received, the review process will begin.  The Charity Committee will review all requests for assistance. You will be contacted by a charity committee member to gather the information necessary for us to make a decision. Not all cases will be approved.  You will be notified if your request for assistance was approved or declined. In some instances the approval process can take 30-60 days. In the meantime, we recommend that you explore additional resources.

DOWNLOAD or Save a Copy in your own Google Drive **(Please do not request access to edit the linked google sheet.  You need to either Save a Copy or Download only)**
Fill in Both Tabs Completely
Print Both Tabs
